dc.description.abstractΠολλοί που ασχολούνται με την Βιοηθική θεωρούν την έννοια της αυτονομίας ως δεδομένη. Η αυτονομία όμως, μια έννοια που συνήθως θεωρείται ότι εμπεριέχει την ελεύθερη βούληση ή ταυτίζεται από ορισμένους μελετητές μαζί της, απειλείται από τον ντετερμινισμό ή από την σκληρή ασυμβατοκρατία. Αν η ελεύθερη βούληση είναι μια ψευδαίσθηση και οι πράξεις είναι καθορισμένες από προηγούμενες αιτίες, τότε δεν υπάρχει περιθώριο για σεβασμό της αυτονομίας των προσώπων. Ένας αυξανόμενος αριθμός στοχαστών υποστηρίζει ότι δεν υπάρχει ελεύθερη βούληση. Εάν έχουν δίκιο, τότε οι περισσότερες ηθικές έννοιες, συμπεριλαμβανομένης της αυτονομίας, θα καταστούν αβάσιμες. Διερευνούμε ποιες είναι οι απειλές για την ελεύθερη βούληση και οι κύριες απαντήσεις σε αυτές τις απειλές. Παρόλο που οι θεωρίες στη συζήτηση της ελεύθερης βούλησης αφθονούν, όλες παρουσιάζουν σημαντικά προβλήματα. Δεν εκπλήσσει το γεγονός ότι δεν υπάρχει οριστική απάντηση στη βιβλιογραφία. Το ισχυρότερο επιχείρημα κατά της ελεύθερης βούλησης, το οποίο έμεινε ουσιαστικά αναπάντητο, είναι το Βασικό Επιχείρημα του Galen Strawson που δηλώνει ότι τίποτα δεν μπορεί να είναι αιτία του εαυτού του και άρα πραγματικά ηθικά υπεύθυνο. Μια πρόσφατη δημοσίευση του Song υποβιβάζει αυτό το επιχείρημα, αφού οποιαδήποτε τεκμηρίωση υιοθετεί την προσέγγιση του βασικού επιχειρήματος θα συναντήσει το πρόβλημα της άπειρης παλινδρόμησης. Συμπεραίνουμε ότι επί του παρόντος δεν υπάρχει σοβαρή απειλή για την ύπαρξη της ελεύθερης βούλησης, άρα η αυτονομία δεν είναι αβάσιμη έννοια. Ακολούθως, εξετάζουμε με κριτική ματιά μεταξύ άλλων την προέλευση της αυτονομίας και το νόημα της στη σύγχρονη Βιοηθική, τις προϋποθέσεις για να θεωρηθεί ένα πρόσωπο αυτόνομο, την αρχή του σεβασμού της αυτονομίας η οποία θεωρείται ένας από τους τέσσερις πυλώνες της Βιοηθικής και τη διαδικασία ενημερωμένης συγκατάθεσης που αντλεί την αξία της μέσω της αρχής του σεβασμού της αυτονομίας. Συμπεραίνουμε ότι δεν είναι τυχαίο ότι η αρχή της αυτονομίας θεωρείται πρώτη μεταξύ ίσων, καθώς φαίνεται ότι όλες οι άλλες αρχές έχουν στον πυρήνα τους την έννοια της. Τέλος, διερευνούμε τι εννοεί ο Immanuel Kant με τον όρο αυτονομία και πώς σχετίζεται με τη σύγχρονη χρήση της έννοιας. Ακολούθως, συζητάμε τρεις περιπτωσιολογικές μελέτες: για την ενεργητική ευθανασία, τη χορήγηση εικονικού φαρμάκου και ασθενή που αρνείται τη μετάγγιση αίματος.el_GR

dc.description.translatedabstractMany bioethicists take autonomy for granted. But autonomy – a notion usually considered to entail free will or be identical to it, by some scholars – is threatened by determinism or by theories such as hard incompatibilism. If free will is an illusion and all our actions are predetermined by prior events, there is no real room for respect of autonomy and persons. An increasing number of thinkers argue that free will does not exist. If they are correct, then most of our ethical notions, including autonomy, would become baseless. The discussion begins with the investigation of the current threats to autonomy and the main answers to these threats. Admittedly, even though there are many theories in the free will debate, all of them have significant problems, therefore it is hardly surprising that there is no conclusive answer in the literature of the free will debate. The stronger argument against the existence of free will is Galen Strawson's Basic Argument, which states that nothing can be causa sui and therefore nothing can be truly morally responsible. Even though until recently there was no satisfactory answer to the basic argument, a recent paper from Song shows that this argument is not impressive. Any justification that takes the approach of the basic argument would inevitably encounter an infinite regress problem. The conclusion of this section is that there is currently no definite threat for the existence of free will and therefore autonomy is not a baseless notion. In the next chapter, we proceed to examine the roots of autonomy and the meaning we give to this term in modern bioethics. Among others, we examine with a critical eye: the requirements for being an autonomous person, the principle of respect of autonomy which is considered one of the four pillars of medical ethics and bioethics and the process of informed consent which derives its value from the principle of respect of autonomy. We conclude that it is no coincidence that the principle of autonomy has been called the first among equals, as it seems that all other principles have the concept of autonomy in their core. Additionally, we try to elucidate Immanuel Kant’s definition of autonomy and its relation with the modern use of the concept. Finally, we discuss three case studies and their relationship with autonomous choices: the first one discusses active euthanasia, the second one discusses placebo administration and the third one discusses refusal of blood transfusion.el_GR
