dc.description.abstractΕισαγωγή: Στις μέρες μας οι περισσότερες κυβερνήσεις συχνά δέχονται πίεση για περιορισμό στις δαπάνες υγείας, οι οποίες συνεχώς αυξάνονται και αυτό έχει σαν αποτέλεσμα να εμφανίζονται σημαντικά ελλείματα. Έτσι, οι χρήστες καλούνται να πληρώσουν κάποια τέλη υπό μορφή συμπληρωμών στα δημόσια συστήματα υγείας. Πρακτική που είναι παγκοσμίως διαδεδομένη, αλλά αποτελεί μία αμφιλεγόμενη πολιτική ελέγχου της κρίσης υπηρεσιών υγείας. Αυτά τα τέλη χρησιμοποιούνται κυρίως για να μειώσουν τη ζήτηση και έπειτα για να αντλήσουν επιπλέον οικονομικούς πόρους για το σύστημα φροντίδας υγείας. Η διεθνής βιβλιογραφία είναι πλούσια επί του θέματος με υποστηρικτές και πολέμιους, με σοβαρά επιχειρήματα υπέρ και κατά σχετικά με τη σκοπιμότητα και τις συνέπειες από την επιβολή συμπληρωμών. Σκοπός: Σκοπός της μελέτης είναι η διερεύνηση των απόψεων, στάσεων και της συμπεριφοράς των ασθενών αναφορικά με το μέτρο της πληρωμής συμμετοχών στο σύστημα υγείας, αλλά και η εκτίμηση του ύψους των συμμετοχών που καταβάλλουν. Η διερεύνηση δηλαδή του πως οι ασθενείς που προσφεύγουν στις δημόσιες υπηρεσίες υγείας αντιλαμβάνονται τη συμμετοχή τους στο κόστος και μάλιστα υπό συνθήκες οικονομικής κρίσης, τι ποσά δαπανούν, πώς κρίνουν το μέτρο και σε ποιο βαθμό αυτό αλλάζει τη συμπεριφορά τους, όσον αφορά τη χρήση αυτών των υπηρεσιών. Η έρευνα πραγματοποιήθηκε από τον Ιούνιο 2015 έως τον Αύγουστο 2015 στην περιοχή της Θεσσαλονίκης. Για την πραγματοποίηση της μελέτης, χρησιμοποιήθηκε δείγμα ευκολίας Ελλήνων πολιτών, στους οποίους διανέμονταν προς συμπλήρωση το ερωτηματολόγιο κατά την προσέλευσή τους σε ιδιωτικό διαγνωστικό κέντρο της Θεσσαλονίκης με σκοπό την χρήση των υπηρεσιών του. Συνολικά διανεμήθηκαν 220 ερωτηματολόγια εκ των οποίων επεστράφησαν 200 ερωτηματολόγια. Το σύνολο των ερωτηματολογίων που αποτέλεσε το δείγμα της έρευνας αντιστοιχεί σε ποσοστό ανταπόκρισης 90,9%. Αποτελέσματα: Από τη μελέτη που προηγήθηκε, φαίνεται ότι γενικά η επιβολή συμπληρωμών δεν έχει επηρεάσει τη χρήση των υπηρεσιών υγείας. Παρόλο που τα ποσά των συμπληρωμών για μια μερίδα πληθυσμού θεωρούνται υπερβολικά, ωστόσο για το μεγαλύτερο ποσοστό του δείγματος της συγκεκριμένης μελέτης δεν αποτέλεσαν αποτρεπτικό παράγοντα για τη χρήση των υπηρεσιών υγείας. Μάλιστα η πλειοψηφία του δείγματος συμφώνησε στο ότι από τότε που αυξήθηκε το ποσό πληρωμών συνέχισε να πραγματοποιεί επισκέψεις σε ιατρούς δημοσίων υπηρεσιών υγείας χωρίς να σκεφτεί ότι θα πληρώσει. Ωστόσο οι περισσότεροι ασθενείς ζήτησαν από το γιατρό να μην γράψει κάποιο φάρμακο ή κάποια εξέταση γιατί σκέφτηκαν ότι θα πληρώσουν και δήλωσαν πως έχουν υπάρξει περιπτώσεις όπου ο γιατρός τους συνταγογράφησε φάρμακα τα οποία όμως δεν έλαβαν γιατί σκέφτηκαν ότι θα πληρώσουν, χωρίς όμως ποτέ να αγοράσουν κάποιο φάρμακο δίχως συνταγή γιατρού. Σημαντικό εύρημα της έρευνας, αποτελεί το γεγονός ότι οι περισσότεροι συμμετέχοντες (71%) με την εφαρμογή του μέτρου των πληρωμών και την αύξηση του ποσού συμμετοχής υποστήριξαν ότι περιόρισαν τις άσκοπες επισκέψεις στα εξωτερικά ιατρεία του Νοσοκομείου, επίσης υποστήριξαν ότι μείωσαν τη σπατάλη και την κατάχρηση των φαρμάκων, όσο και τις άσκοπες και αχρείαστες εργαστηριακές εξετάσεις σε ποσοστό 68% και 66,5% αντίστοιχα. Από την άλλη μεριά, η πλειοψηφία των συμμετεχόντων σε ποσοστό 71% διαφώνησε με το γεγονός ότι με την εφαρμογή του μέτρου των πληρωμών ή την αύξηση του ποσού συμμετοχής τα χρήματα που εξοικονομούνται θα χρησιμοποιηθούν για την βελτίωση των υπηρεσιών υγείας. Το 91,5% του δείγματός της έρευνας δήλωσε ενήμερο για την καταβολή του ποσού των συμπληρωμών. Από τις συσχετίσεις που πραγματοποιήθηκαν μεταξύ των δημογραφικών χαρακτηριστικών και κάποιων εξαρτημένων μεταβλητών, αρχικά, βρέθηκε ότι λιγότερο ικανοποιημένοι με τις υπηρεσίες υγείας εμφανίζονται οι άνδρες σε σχέση με τις γυναίκες, τα πιο νεαρά άτομα σε σχέση με τα άτομα μεγαλύτερης ηλικίας οι ιδιωτικοί υπάλληλοι, οι συνταξιούχοι και οι άνεργοι σε σχέση με τους υπολοίπους και όσοι πάσχουν από κάποιο χρόνιο πρόβλημα υγείας σε σχέση με όσους δεν πάσχουν. Επίσης, βρέθηκε ότι η αυτοεκτίμηση του ύψους της καταβολής των τελών δεν συσχετίζεται με κανέναν δημογραφικό παράγοντα. Τέλος, προέκυψε ότι με επιβολή τελών σε όλες τις υπηρεσίες υγείας λιγότερο συμφωνούν οι γυναίκες σε σχέση με τους άνδρες και όσοι πάσχουν από κάποιο χρόνιο πρόβλημα υγείας σε σχέση με όσους δεν πάσχουν. Συμπεράσματα: Από τα ευρήματα της παρούσας έρευνας προκύπτει το γεγονός ότι η εφαρμογή του μέτρου των συμπληρωμών στο δημόσιο σύστημα υγείας της Ελλάδας δεν φαίνεται να αποτελεί εμπόδιο στη χρήση των υπηρεσιών υγείας. Αυτό το μέτρο αν και παρουσιάζει αρκετές προοπτικές για βελτίωση των συνθηκών στον τομέα της υγείας, ωστόσο είναι αναγκαία η προσεκτική εφαρμογή του και η ακόμα προσεκτικότερη διάρθρωσή του.el_GR

dc.description.translatedabstractIntroduction: Nowadays most governments often receive pressure to limit health spending, which are constantly increasing and this results to show significant deficits. Thus, users are required to pay a fee in the form of co-payments on public health systems. Practice is universally widespread, but is a controversial political control of crisis health services. These fees are used primarily to reduce demand and then to raise additional financial resources for the health care system. The international literature is rich on the subject with supporters and opponents, with strong arguments for and against on the feasibility and consequences of the imposition of co-payments. Aim: The aim of this study is to explore the opinions, attitudes and behavior of patients with respect to the measure of the equity payment to the health system, but also to assess the amount of investments made. How do those patients who turn to public health services perceive their participation in the health expenditure in an era of economic crisis, how much do they spend, what is their opinion regarding the reform and to what extent it changes their behavior in the use of such given services. The period of time the survey was conducted was from June 2015 to August 2015 in the area of Thessaloniki. The people who filled in the given questionnaire, were only those who visited a certain private diagnostic center in Thesaloniki and used its facilities. In total 220 questionnaires were distributed and 200 of those returned. Therefore the sum of the questionnaires that were taken into consideration was of the percentage of 90,9%. Results: The study showed that imposing co-payments does not really affect the use of health services. Although the amount of co-payments for a proportion of the population significant burden, in this study it appears they were not discouraged using all health services. Indeed the majority of the sample agreed that despite the increased amount of payments, they continued to visit a general hospital doctor or dentist or a specialist doctor in a hospital without thinking about the cost. On the other hand many patients have asked the doctor not to prescribe some medicines or diagnostic laboratory tests because they couldn’t afford the extra payments and they made the statement that there have been instances when their doctor actually prescribed drugs but they didn’t buy them. At the same time, what is interesting is that they have never bought a medication without a prescription. An important finding of this survey is the fact that most participants (71%) after the implementation of the additional payment measures have reduced the unnecessary visits to outpatient clinics. Furthermore overconsumption of drugs and unnecessary laboratory tests were reduced as stated by the 68% and 66,5% of the respondents respectively. On the other hand, the majority of participants (71%) disagreed with the fact that the implementation of payment measures or the increase of the participation fees will actually save resources which in turn will be used to improve health services. According to the findings, 91.5% of the survey sample were well informed about the amount of co-payments. Men appeared less satisfied with health services than women. In general young people were less satisfied than older people. Private employees, pensioners and the unemployed were more dissatisfied compared to the rest of the population. Those who suffer from a chronic health problem feel less comfortable than those who do not have any health problems. It was also found that the estimated amount of the copayments, is not related to any demographic factor. Finally, it was found that women and patients suffering from a chronic disease agree less than men and people without a health problem in imposing co-payments to all health services. Conclusions: The findings of this research show that the implementation of the measure-payments in the public health system in Greece is not an obstacle to an extended use of health services. This measure although has several points for improving, nevertheless it is necessary in the health sector, taking into consideration the necessary the careful implementation of an even better restructuring.el_GR
